We complete the Standard Medical Report Form (SR-LC-080 05-02) used for routine driver medicals. We do not complete specialized assessment forms.
Forms we do not complete include:

When do commercial drivers need a report?
Classes A, B, C, D, E and F must submit medical reports when applying and periodically when renewing.

We focus on the Ontario Standard Medical Report Form. Please call the clinic before booking to confirm whether your specific out-of-province commercial form can be completed.
No. We do not complete specialized forms for substance abuse, neurological conditions or cardiovascular disease. We complete the Standard Medical Report Form SR-LC-080 05-02.
Class G and M drivers generally need a medical exam when the Ministry requests one, including when a driver does not meet the standard peripheral vision requirements.
We offer appointments six days a week: Monday through Saturday, with hours varying by day. The Sunday call centre is open even though the clinic is closed.
Standard forms are generally completed during your appointment when all requirements are met. Extra medical information or follow-up testing may occasionally be required.
